DHS Disability Services – Scoping Report on Best Practice Approaches to Broad Individualised Planning for People with a Disability 2008
LDC Group undertook an analysis of national and international models in relation to best practice approaches for the delivery of broad, individualised planning (separate to service provision) for people with a disability for the Department of Human Services (DHS), Disability Services. The primary aim of the project was a review of relevant publications and websites, and the preparation of this report which focuses on:
- Key learning’s and findings from the implementation of broad, individualised planning
- Best practice approaches to providing individualised planning
- Analysis of the benefits and risks of locating individualised planning within or outside of government
